HOW IT WORKS

Consultation and Planning

When you come to a consultation at ROXY Plastic Surgery, Dr. Grawe will examine you and determine if you are a candidate for breast augmentation. You will discuss how augmentation works and how to make some important decisions about your surgery. Where to place the scar? Silicone or saline? What size is best for you? Above or below the muscle?

Implant Location:

The implant is usually placed partially below the chest muscle.  This muscle protects the implant from poor scarring (capsular contracture/link to that section) and makes the augmentation look very natural.

Scar Location:

The breast augmentation scar can be placed in a few different locations:

Inframammary fold: This scar sits beneath the breast in the crease and is completely hidden unless you pull your breast up to show it.
Periareolar: This scar sits at the junction of the dark skin around your nipple and the breast skin on the lower half of the areola.  It is on the front of the breast but makes a very faint scar at this location.

Axillary Crease: This scar is in a crease in your armpit.  The advantage of this location is that there is no scar on your breast.

Types of Breast Implants – Saline vs Silicone

There are 2 main types of implants – Silicone and Saline. Dr. Grawe will help you decide which one is best for you. At your consultation, you will have a chance to see and feel the implants. Here are some of the main points about each:

Silicone – Advantages:
  1. Looks and feels very much like a natural breast
  2. If it ruptures, it does not cause a medical problem and you may not even know it unless you get an MRI to show the rupture.
  3. Less likely to have rippling or wrinkling
Silicone – Disadvantages:
  1. The FDA requires that you are at least 22 years old or are having reconstruction to have this implant.
  2. It is more expensive than a saline implant.
Saline – Advantages:
  1. Less expensive than silicone.
  2. You may have this implant if you are 18 years or older or require reconstruction.
  3. If the implant ruptures, saline (salt water) is released and your body naturally absorbs it.
Saline – Disadvantages:
  1. If it ruptures, you go completely flat on that side and will need another surgery to remove it and place a new implant.
  2. There may be visible or palpable wrinkling or rippling.

How to Choose the Right Size of Breast Implants

There is no way to correlate implant sizes (cc’s) with breast size (bra size) because there are so many factors involved including your ribcage size, your current breast size, the strength of your muscle, and the type of implant used. The best way to decide implant size is to have an idea of what bra size you want to be, and at your consultation, Dr. Grawe will help you choose what size is right for your specific frame and what you are hoping to achieve.

We now have Crisalix 3D imaging! This technology makes it so easy to visualize what you will look like after breast augmentation. A front and side view picture will be taken of you and converted to a 3D image. You will then be able to “try on” different size implants and see what you would look like with and without clothes on! Not only is it fun to see what you would look like, but it’s complimentary and only takes a few minutes.

The Procedure

Breast augmentation is an outpatient surgery, so you can go home the same day. The surgery takes about one hour. You will have a small scar which can be placed under your breast fold, around your areola (the dark part around your nipple) or away from your breast in your armpit.  An implant is placed below your chest muscle.  Your stitches will be dissolvable and underneath your skin, and you will wake up in a surgery bra.

Recovery

Most people have some discomfort after surgery. This is easily taken care of with pain medicine which will be prescribed for you by Dr. Grawe. You will notice a difference right away. Although you will have swelling after surgery, you will look great in clothes immediately! Before you know it, you will have fuller, soft, natural looking breasts and look great in and out of clothes!

You will notice a difference immediately after surgery, but you will be very swollen and the results will improve with time as the swelling goes down. At first, you look great in clothes, but it will take about a month for the swelling to go down enough to look great out of clothes. In three months, your results will be natural and soft.

You should not lift anything heavier than a gallon of milk for at least 2 weeks after surgery. You should walk around immediately after surgery and NOT stay in bed. This is important to prevent blood clots. You may remove your outer dressing and shower 48 hours after surgery. You should eat a healthy diet and as much protein as possible to promote good healing.

You may drive and return to work when you are no longer taking prescription pain medicine. Most people are able to return to a light duty job about 1 week after surgery.

Remember that everyone is different. Recovery time and progression is not the same for everyone. The times stated are typical for healthy individuals healing from this surgery. The normal progression of healing may be different for those with health problems, smokers, and those who have unforeseen complications after surgery.

Results

After breast augmentation, you will enjoy fuller breasts which will fit into your bra, clothes, and bathing suit better. Your scar will be hidden and you will have a great breast shape. Breast augmentation will give you confidence in how you look… in and out of clothes! Individual results may vary.

Risks

With every surgery, there are risks involved including bleeding, infection, blood clots, pain, scarring, delayed healing and need for further surgery. Here are some potential Breast Augmentation risks:

Infections in the presence of an implant are harder to treat than infections in normal body tissues.  If an infection does not respond to antibiotics, the implant may have to be removed.

Scar tissue, which forms internally around the tissue implant, is normally soft and you cannot feel it but, it can tighten and make the breast round, firm, and possibly painful. Excessive firmness of the breasts can occur soon after surgery or years later and is not predictable. This problem often requires surgery to treat.

The implant may be seen or felt mostly on the bottom of the breast or on the side where the muscle doesn’t completely cover the implant. This can give a wrinkled look and occurs most often in very thin patients with very little breast tissue to start out with. This occurs less often with silicone implants.

Implants of all types are manufactured products and can fail.  They can deflate which causes them to change shape.  This is repaired with further surgery to replace the implant.
